
Student/Employee Forms & Tuberculosis Skin Testing
Starting a new program or job should be exciting — not stressful. Many colleges, universities, and healthcare placements/employers require students/employees to provide proof of immunizations, Tuberculosis skin testing, and official health forms before they begin. That’s where we come in. Our goal is to help you meet your requirements quickly, so you can focus on your education and future career.

Schedule a convenient time with us using our online booking system or call 705-963-0525 to make an appointment for a Tuberculosis skin test — no referral needed. We recommend booking at least 4–6 weeks before your program deadline, especially if a two-step Tuberculosis test is required.
You will receive a link to create an account and upload your student health form(s) and past immunization records to your portal. If you’re unsure what’s missing, don’t worry — we’ll help you sort it out. Please use for immunization records request or call Algoma Public Health.
Our Registered Nurse will review your records and compare them against your school’s requirements. If you’re missing vaccines, our NP will order blood work to determine if you have immunity to the diseases the school is requesting. Once your blood work results are received, we will determine if you need any immunizations. If you need immunizations, depending on eligibility, we can provide them at your appointment.

For one-step testing, you’ll need 2 visits: placement and reading (48 to 72 hours later).

For two-step testing (often required for healthcare programs), you’ll need 4 visits over 2–3 weeks.
We’ll guide you through the schedule so you don’t miss your deadline.
Once your blood titers, vaccines and Tuberculosis testing are up to date, we’ll complete and sign your forms as required by your school. Most forms are finalized at the appointment or immediately after your Tuberculosis test results are read.
We’ll give you a copy of your completed forms and any updated immunization records so you can submit them to your program with confidence.
