Student/Employment Forms
FAQ
Do you complete student health forms for school or college programs?
Yes. We assist students with required health forms for nursing, healthcare, college, and university programs, including immunization records, blood requisitions and TB testing.
Can you provide documentation for my school once testing is complete?
Yes. We will provide official documentation for your program’s requirements once all tests and vaccinations are completed.
What is a TB skin test?
A TB skin test helps determine if you have been exposed to tuberculosis. A small amount of testing solution is injected under the skin, and the site is checked after 48–72 hours.
How much notice do I need to book a TB test before my forms are due?
We recommend booking at least 4–6 weeks before your deadline, especially if a two-step TB test is required, to allow enough time for completion. Also consider that you must wait at least 4 weeks after a TB test before receiving any live vaccines.
How many visits are required for a TB skin test?
Two visits: one to place the test, and one to “read” the result after 48–72 hours. Some students may require a two-step test, which involves four visits.
Do you order blood work to check immunity and update student immunizations as well?
Yes. During your appointment we can review your immunization record and order blood work to check immunity before providing any missing vaccines required for your program. We do not provide publicly funded vaccines.
